INSURANCE:

Insurances Accepted: Aetna, Blue Cross/Blue Shield/Regence, Cigna, Medicaid, Medicare, Select Health, United Health Care/Optum

I am paneled with most insurance companies. I also provide out-of-network services at a fee-rate directly to you. As a courtesy, I can provide you with a "superbill" that you may submit to your insurance carrier for possible reimbursement. This does not apply to Medicare/Medicaid. However, reimbursement may be denied by your insurance depending on your specific plan and coverage. Therefore, you are encouraged to call you insurance carrier to request information regarding your policy and coverage for mental health information. They may ask for the following provider information:

NPI number: 1366950933

UT Psychologist License: 13409800-2501

ID Psychologist License: PSY-203726

You may choose a private pay option. There are benefits to paying privately instead of using health insurance including:

  1. Privacy: There are barriers to privacy when a third-party payer is financially responsible for treatment/service costs, and there are restrictions on treatment and assessment. For example, some insurances only approve a set amount of appointments, such as six appointments per year, or only approve certain therapeutic approaches which limits provider's flexibility in using different approaches that best suit each client. Some insurance companies limit the amount of time a provider can spend providing assessment services, which places limits on the potential benefits of services. With that being said, I will not recommend further therapy or assessment if it is not necessary in my professional opinion, and of course, you are always welcome to seek a second opinion or opt out of services.

For many people, privacy is an increasing priority and a growing concern. For those who wish to keep their mental health records private for employment concerns, future medical or insurance concerns, or for those who wish to minimize records for their treatment, a private fee-for-service option is preferred. There are increasing incident rates of hacking and breaches of confidential information within large corporate data bases. By keeping your records private, there is minimal risk of your health information getting stolen or hacked.

  2. Diagnosis: Health insurance companies require diagnoses in order for providers to receive reimbursement. Diagnoses are not always appropriate since some individuals seek therapeutic services for relational concerns, grief, or other reasons that are not considered to be "diagnosable" or "disorders." I am not able to provide a diagnosis if you don't meet criteria as this is unethical, dishonest, and is also considered insurance fraud. This means that I can provide services to individuals who may not otherwise qualify for services if insurance was being billed. When offering private pay services, a diagnosis is not required, and it provides more privacy and protection for individuals who wish not to have a diagnosis listed in their records for employment eligibility reasons. With that being said, in order to receive counseling services, there has to be a problem or set of symptoms you're hoping to improve or a goal you're striving for.

  3. Coverage: Lastly, some health insurance companies do not provide counseling and/or psychological assessment coverage. This means that your insurance would possibly deny any claims I would submit on your behalf, and you would be required to pay out-of-pocket regardless.

FEES: My fees are listed below. Fees are expected to be paid at the time that services are rendered.

Counseling:

The frequency of counseling appointments are agreed upon by myself and the individual client. Typically, clients are seen once a week; however, clients may also be seen every other week or on a monthly basis, depending on presenting concerns and need.

Initial Appointment (60 min): $250

Individual Counseling (55 min): $200

Family/Couples Counseling (60 min): $200

Assessment:

Testing administration fees are set at a higher rate due to the cost of materials (tests, protocols, technology, etc).  The cost of the assessment depends on the time required to complete the evaluation. The duration of test administration/scoring, interpretation, and report writing are dependent upon what the assessment is for and varies case by case. I would be able to provide you with a more accurate estimate of cost during an initial appointment.

Initial Appointment (60 min): $250

Testing Administration/Scoring (30 min): $125

Test Interpretation/Report Writing/Providing Results to Clients (60 min): $200
